Claudication is the pain in the upper and lower limbs caused by too little blood flow, particularly during exercise. It is the indication of peripheral artery disease.

SIGNS AND SYMPTOMS OF CLAUDICATION  

  • Aching pain in extremities
  • Burning feeling
  • Weakness
  • Numbness
  • Pain relieved by rest
  • Cool skin
  • Slow-healing wounds

CAUSES OF CLAUDICATION  

  • Peripheral artery disease

PATHOPHYSIOLOGY OF CLAUDICATION  

In peripheral artery diseases like atherosclerosis, plaque deposits occur inside the arteries leading to a reduction in the blood flow to the limbs. It begins during walking, exercise etc and ends with rest hence the pain is intermittent.

DIAGNOSIS OF CLAUDICATION   

  • Laboratory tests
  • Musculoskeletal examinations
  • Neurological examinations
  • Magnetic resonance imaging (MRI)
  • X – rays
  • Angiography
  • Exercise testing
  • Doppler ultrasound

 

TREATMENTS FOR CLAUDICATION  

  • Medications –Analgesics
  • Treatment of underlying pathology
  • Surgical management – angioplasty, vascular surgery

 

PROGNOSIS OF CLAUDICATION  

  • Prognosis is based on the duration of onset of pain and the condition of the patient
  • Surgical and non-surgical remedies are effective in the management of claudication.

COMPLICATIONS OF CLAUDICATION  

  • Non-healing wounds
  • Constant pain may affect the day to day activities
  • Skin darkening in the affected extremities
  • Gangrene formation

 

CLAUDICATION AND AYURVEDA

  • In Ayurveda, claudication symptomatologically falls under Vatavyadhi or a Vata dosha imbalance disorder. Vata vyadhi can be correlated to neurological or neuromuscular disorders. The causes and pathogenesis of diseases caused by vitiation of Vata dosha, in general, are the exclusive cause and pathogenesis of pain, numbness, weakness.
  • Peripheral artery diseases are managed based on ‘gambhira vata rakta’ treatment.

 

NIDANA- AYURVEDIC CAUSES OF CLAUDICATION  

  • Excess consumption of ununctuous, cold, oily food
  • Lack of physical exercise (Alpa vyayama)
  • Suppression of natural urges (vegas)

 

PURVAROOPAM- AYURVEDIC PREMONITORY SYMPTOMS OF CLAUDICATION  

  • Dull intermittent pain in the affected limbs

 

SAMPRAPTI – AYURVEDIC PATHOGENESIS OF CLAUDICATION  

  • Vata, pitta and kapha dosha move through all channels of circulation. Due to the subtle nature of vata it impels the remaining two doshas.
  • The aggravated vata having provoked these two doshas, exacerbated and fills up the empty channels and moves greatly inside them or by getting enveloped by the other doshas.
  • The aggravated vata resides in the limbs associated with sira and dhamanis and produces different kinds of aches and pains in limbs.

  • Toda – aching pain
  • Sada – weakness
  • Syavatha – skin discolouration

DIET AND BEHAVIOUR FOR CLAUDICATION  

  • Avoid food items that produce vata dosha imbalance like cold and dry food, hard to digest. Because vata imbalance aggravates the symptoms.
  • Consume easily digestible food items, cow’s milk
  • Avoid heavy exercises which further aggravate the conditions.
  • Oil massage on the whole body is advised. That alleviates vata dosha.
  • Take proper rest.

 

YOGA FOR CLAUDICATION  

  • Nadi Shuddi Pranayam – calms the mind and gets relief from aches and pains

 

The patient needs to be seated in a meditative posture with the head and spine erect, and the body relaxed. The patient has to close one nostril (e.g. left nostril if using the right hand and vice versa) with the thumb and exhale completely through the other nostril. Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.

 

  • Savasana – relaxes body and mind
