Ayurvedic Treatment for Charcot Marie Tooth Disease

Introduction

These are a group of genetic disorders that cause peripheral nerve damage (in arms and legs). The condition which is also termed as hereditary motor and sensory neuropathy, causes loss of sensation mainly over feet, legs, arms and hands. The musculature of these parts will become weaker and thinner. The signs and symptoms can be evident from adolescence or early childhood, but may be delayed in some person.

Generally, the symptoms start on legs or feet. Weak legs and feet, frequent falls, impaired gait, reduced muscle bulk on calves and feet, decreased/ loss of sensation over legs and feet, high foot arches, footdrop, hammertoes etc are the usual signs and symptoms, though the severity is different from person to person. As a progressive disorder, the symptoms may spread to arms on a gradual manner.

Causes of Charcot Marie Tooth Disease

It is a genetic condition and is caused by gene mutations in arms, legs, feet and hands which lead to destruction of nerve cells or myelin sheath resulting in interrupted signal transmission from brain to the peripheral nerves.

Family history is the major risk factor in Charcot-Marie-Tooth-Disease. Other diseases causing neuropathies (eg: diabetes) usually present with similar symptoms and these will definitely worsen the condition. Also, some medicines such as chemotherapy drugs can make the symptoms worse.

Treatment of Charcot Marie Tooth Disease

As it is a genetic problem, it is of no cure. Pain medications are recommended in case of pain. Rehabilitation techniques like physiotherapy (stretching exercises) and occupational therapy are beneficial. Orthopaedic devices (leg and ankle braces or splints, boots, thumb splint etc) are useful for getting additional support. To correct your gait, shoe-inserts or customized shoes can be used. Surgery is an option for correcting foot deformities.

Complications of Charcot Marie Tooth Disease

Most cases of CMT are not life-threatening or fatal. The severity of complication may vary from person to person. The major ones include abnormalities of foot, muscle weakness, difficulty in walking, chances of fall (bad signalling from brain to feet), injuries or infections to areas of body with less sensations and problem with swallowing or breathing if the related muscles are affected.

Preventive measures of Charcot Marie Tooth Disease

In order to prevent the complications, do regular exercise, improve body stability (use a walker while walking), ensure proper lighting at night times to avoid falling. Foot care is very important. Regularly check for any calluses, wounds, ulcers and infections (these may be overlooked as there is no pain sensation to warn you). Neatly cut the nails regularly to avoid infections, use properly fitting shoes to protect the legs. Try to wear boots for ankle support. Customized shoes are also advised in the case of foot deformities like hammer toe.

Ayurvedic Concept of Charcot Marie Tooth Disease

As the condition is a debilitating disease of the nervous as well as locomotor systems, the main Dosha involved here is Vaata. The deranged Vaata (either abnormally increased, decreased or blocked out of its normal route) causes structural deformities and functional anomalies in body parts especially extremities. The pathogenesis can happen in two ways, with or without the involvement of Kapha dosha. In teens or young adults, and in people with other debilitating neuropathic diseases like diabetes, metabolic waste products or Ama covers up or blocks the body channels including nerves & circulatory system. In chronic cases, the Vaata dosha alone manages to rule the pathogenesis with abnormal increase and derangement of itself, leading to muscular atrophy and nervous degeneration. Neuropathies are the deranged Vaata going to various parts of the body and causing Vaata kopa lakshanas like numbness and altered sensation.

Ayurvedic treatment for Charcot Marie Tooth Disease

The Ayurvedic management for each individual may vary according to the clinical presentation but the priority is to control and regulate to ensure a normalised Vaata in its normal course & functions. The herbal medicines, formulations and therapies are focused on removing the blocks in Vaata doha’s course in body channels and preventing its occurrence further.

Ayurvedic Panchakarma therapies are administered under expert supervision, after considering the age & strength of the patient. Usage of herbal & medicated sesame oils and ghee preparations are advised first especially in Vaata alone is the vitiated Dosha or with Pitta association. After using such ghees & oils both as internal medicines & external therapies, sweat-inducing therapies are administered. By the combination of such oleation-sudation therapies, the metabolic wastes & toxins that cause obstruction to channels get melted and flow towards Koshtha (intestinal trat). This should be cleaned up and expelled from the body by Ayurvedic Panchakarma therapies. The following therapies are used in most of the CMT cases for cleansing and detoxification of the body.

Vamana – therapeutic and induced vomiting with herbal formulations

Virechana – therapeutic and induced purgation with herbal formulations

Two types of Vasti – herbal enema procedures with various formulations

Nasya – herbal nasal drops

Diet, Yoga & Pranayama

People with CMT should be very careful about diet and regimen, as Vaata is very much depended on these factors. A healthy diet and stress-free daily life can help the angry Vaata under control, leading to less severe signs & symptoms. Diet rich in Calcium and micro vitamins, fibres and healthy fats are very important. But it is more important to eat according to the Agni and hunger schedule. Late breakfast and late dinner should be avoided like otherbad food habits like overeating.

Yoga is a way to heal all systems of the body and the body as a whole. Certain yoga postures along with mental & emotional balancing techniques can help improve the body’s own defence & healing mechanisms.

Side effect management of Charcot Marie Tooth Disease by Ayurveda

In many cases, medicines used for signs of CMT include steroids & others that are harmful when used long-term. Ayurveda can help in the detoxification of the body. Ayurvedic diet, lifestyle and medicines can also improve the body’s power to digest absorb and assimilate both food & medicines. In that way, the body with Ayurveda regimen will be stronger to withstand the adverse effects of both the disease and medications for it.
