Ayurvedic Treatment for ADENOMYOSIS

Ayurvedic Treatment for Adenomyosis focuses to fix the various factors influencing the imbalance of female hormones. Adenomyosis is an abnormal condition of the uterus in which the endometrial tissue that normally lines the uterine cavity grows into the muscular wall of the uterus. This abnormally grown tissue goes into the menstrual cyclic changes like thickening, breaking down, and bleeding. This results in an enlarged uterus and heavy menstrual bleeding associated with pain.

Signs and Symptoms

Occasionally Adenomyosis goes undiagnosed due to mild symptoms. But in conditions where the condition is serious, it may have signs and symptoms like,

  • Dysmenorrhea: severe cramping pain in the lower abdomen and pelvis associated with menstruation
  • Prolonged heavy bleeding
  • Dyspareunia- the pain is felt during the sexual intercourse
  • Abdominal pressure and bloating may be seen in some cases.
  • The size of the uterus increased in some cases.

Causes

Adenomyosis is a common condition and prior uterine surgery for C section, fibroid removal, dilatation, and curettage, etc. act as the risk factors. The exact cause of Adenomyosis is still unexplained. The frequently suspected theories include,

  • Repeated childbirth
  • Vigorous curettage
  • Excess effect of estrogen

Pathophysiology

Associated with the extension of endometrial glands and stroma beneath the endometrial –myometrial interface. Due to the absence of submucosa, endometrial glands are in direct contact with the myometrium. It forms nests deep within the myometrium leading to myometrial hyperplasia around the endometriotic foci. Trauma to the deeper endometrium is also an etiological factor.   

Diagnosis

  • History taking, signs, symptoms
  • Pelvic Examination- uniform enlargement of the uterus.
  • Ultrasound imaging and color doppler
  • MRI of uterus
  • In some cases endometrial biopsy

Treatments

The symptoms usually subside by menopause. The treatment choice includes,

  • Hormone regulators – Estrogen- progesterone pills, hormone-containing vaginal rings, Levonorgestrel releasing IUS, etc helps to improve menorrhagia and dysmenorrhea.
  • Anti-inflammatory drugs to control pain
  • Surgical management includes Adenomyomectomy, Hysterectomy- in severe conditions if no relief is found with medications.

Prognosis

It is a benign but progressing condition. They are likely to have endometriosis, uterine fibroids and may lead to infertility.

Complications

They are likely to have endometriosis, uterine fibroids and may lead to infertility.

Preterm labor and premature rupture of membranes.

Disease and Ayurveda

The symptoms of Adenomyosis resembles to the symptoms described for Vaatiki yoniroga and also in Asrigdhara.

Nidana- Causes

The cause of yoni rogas is said as,

  • Vishamangasayana (inappropriate postures during coitus)
  • Excess sexual acts
  • The menstrual blood if vitiated with Vata, pitta, kapha doshas
  • Sexual acts using foreign materials.
  • Irregularities of semen and ova
  • Excessive pitta vitiating diet may cause Asrigdhara(Menorrhagia)

Purvaroopam- Premonitory Symptoms

No premonitory symptoms were described.

Lakshana Signs and Symptoms

  • Pricking pain in vagina, numbness, crawling like pain.
  • Hardness in vagina
  • Bleeding resembling blackish frothy blood
  • Pain in the pelvic region
  • In Asrigdhara there will be excessive bleeding may be associated with clots.

Prognosis

Difficult to cure the disease if it is chronic. Timely treatment can help to control the symptoms.

Chikitsa- Treatment

Vata samana Oushadhas followed by Snehapana, Swedana (sudation), Vasthi (enema) can be used in the management.

Discontinuation of the causes of disease are also necessary.

  • Pachanamrutham Kashayam
  • Hinguvachadi Churna
  • Vyoshadi Guggulu
  • Eranda Sukumaram
  • Vasaguluchyadi Kashayam

  • Sukumaram ghrita (snehapana)
  • Bala thaila – for paana and vasthi (enema)
  • Pushyanuga choorna with honey in case of excess bleeding
  • Musalee khadiradi kashaya can also be given in excess bleeding

Home Remedies

  • Mild heat in the abdomen and pelvis to prevent pain during maturation
  • Turmeric is good to prevent inflammation
  • Water boiled with fenugreek seeds, cumin seeds are good
  • Light easily digestible and hot food is desirable.

Diet

Brown rice, wheat, oats, Apple, papaya, pineapple, cabbage, cucumber, green leafy vegetable, carrots, etc. are to be consumed.

Maida, frits like mango, watermelon, citrus fruits, onion, garlic, etc. are to be avoided.

Yoga

Yoga posture which helps in maintaining the health of pelvis organs can be chosen.

  • Upavistha konasana
  • Navasana/naukasana…
  • Viprit karni.
  • Anulom viloma pranayama
